Class I indications for primary PCI:
- recent myocardial infarction + chest pain from less than 12 hours + persistent ST-segment elevation,
- recent myocardial infarction + chest pain from less than 12 hours + previously undocumented left bundle branch block,
- recent myocardial  infarction + chest discomfort from less than 12 hours + persistent ST-segment elevation,
- recent myocardial  infarction + chest discomfort from less than 12 hours + previously undocumented left bundle branch block.

Class IIa indications for primary PCI:
- recent myocardial infarction + chest pain from more than 12 hours + persistent ST-segment elevation,
- recent myocardial infarction + chest pain from more than 12 hours + previously undocumented left bundle branch block,
- recent myocardial  infarction + chest discomfort from more than 12 hours + persistent ST-segment elevation,
- recent myocardial  infarction + chest discomfort from more than 12 hours + previously undocumented left bundle branch block.

Class IIb indications for primary PCI:
- recent myocardial infarction + chest pain before more than 12 hours and less than 24 hours + persistent ST-segment elevation,
- recent myocardial infarction + chest pain before more than 12 hours and less than 24 hours + previously undocumented left bundle branch block,
- recent myocardial  infarction + chest discomfort before more than 12 hours and less than 24 hours + persistent ST-segment elevation,
- recent myocardial  infarction + chest discomfort before more than 12 hours and less than 24 hours + previously undocumented left bundle branch block.
